Job Summary:
•Position requires specific knowledge of the symptoms and conditions of the patient under the direction of the patient’s physician and a written plan of treatment specifically designed for the individual patient.
•Deliver comprehensive nursing care to patients in their homes, ensuring adherence to established protocols and standards.
•Assess, interpret, document, and communicate patients’ symptoms, reactions, and needs effectively.
•Develop and implement individualized care plans in collaboration with the healthcare team, addressing physical, emotional, and spiritual needs.
•Provide education and support to patients and families, empowering them in their care journey.
•Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
•Collaborate with interdisciplinary teams to enhance the quality of care delivered to patients in the home health and hospice setting.

Essential Skills:
•Strong understanding of home health and hospice care principles and practices.
•Ability to perform comprehensive assessments and develop care plans tailored to individual patient needs.
•Phlebotomy proficiency
•Excellent communication skills for effective interaction with patients, families, and healthcare teams.
•Proficient in medication management and understanding of pharmaceuticals for patients of all ages.
•Ability to perform emergency interventions and understand advanced life support protocols.
•Knowledge of infection control practices and proper documentation standards.
•Strong organizational skills to manage multiple patient cases and prioritize care effectively.
•Ability to establish and maintain effective working relationships with patients, families, and colleagues.

Minimum Qualifications:
•Associate degree in nursing from an accredited nursing program.
•Valid Registered Nurse license issued by the State of California.

Experience:
• One (1) year med/surg/Home Health/Hospice experience.

Licensure / Certifications / Registrations:
•Current California RN license.
•BLS Healthcare Provider Certification.
•Valid driver’s license and reliable vehicle for transportation to patient homes.
•Proof of auto insurance coverage.

Preferred:
• Home Health Care Nurse Certification preferred.
Pain management Certification preferred.
Oasis and/or Kinser experience preferred at all levels
•Experience in home health and hospice care
•Pediatric Advanced Life Support (PALS) and/or Advanced Cardiac Life Support (ACLS) within six months of hire.
