Job Summary:
• Analyzes patient data and records for accuracy, completion and compliance with all hospital bylaws, State and Federal standards.
• Receives and processes patient records and loose paperwork.
• Scanning of all inpatient and outpatient medical records into EHR.
• Organize and maintain patient data for clinical databases and registries ensuring release of information is completed according to policy.
• Retrieves EMR and paper records for internal and external requests.
• Maintains confidentiality and adheres to HIPAA guidelines.
• Provides technical and administrative assistance as needed. Provides physician support. Answers telephones and operates basic office equipment.
Essential Skills:
• Strong Computer knowledge.
• Accurately type 50 words per minute.
• Handle multiple phone lines and accurately record messages.
• Ability to understand and carry out written, oral, and/or graphic instructions.
• Ability to operate office equipment.
• Ability to read and interpret documents accurately.
• Ability to analyzes and distribute medical charts. Locate, organize, and file documentation in patient’s medical record.
• Ability to reviews medical records for quality standards, lists all incomplete documentation for notification of provider. Identifies and reports any inaccuracies in patient data.
• Understands and complies with policies and procedures related to HIM, including confidentiality, amendment of medical records, release of information (ROI’s), patient’s rights, medical records as legal evidence up to and including informed consent.
• Maintains, delivers, prepares, and stores the patient medical records as required by established policies and procedures.
• Correctly prepares records for new patients and files loose medical reports, correspondence, patient data into files correctly and delivers requested medical records in a timely manner.
• Copies medical records with an understanding of the legal implications and responsibilities related to the copying of medical records.
• Organizes and prioritizes incoming requests for medical records to accommodate the required turnaround time.
• Accurately applies knowledge of medical terminology, anatomy, physiology, diagnostic and therapeutic procedures performed during patient encounters related to reporting requirements. Uses reference materials appropriately and efficiently to facilitate accuracy in assigning labels.
• Meets quality and productivity standards and deadlines established by Director and or Manager of HIM.
• Establish and maintain effective work relationships with those contacted in the performance of your duties.
Minimum Qualifications:
• High school graduate or equivalent.
• One or more years of clerical experience, preferably in a medical setting.
• Experience with record maintaining both paper and electronic, preferably in a medical and or hospital setting.
• Successful completion of an Associate’s Degree program preferred, but not required.
