RN Case Manager-Home Health/Hospice - Home Health/Hospice
I. Job Summary: Provides skilled nursing or hospice care to patients in their place of residence under the direction of the patient's physician and a written plan of treatment specifically designed for the individual patient.

Employees in this position work from broad policies and on general objectives, referring specific matters to supervisor only when interpretation or clarification of organizational policies is necessary.

II. Minimum Qualifications:
A. Education
1. High school graduate or equivalent.
2. Successful completion of an accredited school of registered nursing.

B. Experience
Level I One (1) year med/surg experience.
Level II Minimum of two (2) years med/surg experience, and one (1) year Home Health/Hospice experience.
Level III Minimum of two (2) years experience in med/surg, or public health, and two (2) years Home Health/Hospice.
Level IV Minimum of two (2) years experience in med/surg, or public health, and three (3) years Home Health/Hospice.

C. Licensure / Certifications / Registrations
All Levels
1. Current valid California RN license or Interim Permit.
2. Valid Basic Life Support/Healthcare Provider certification.
3. Current valid California Driver's License
Level III
1. Same as level I, plus Oasis Certification

Level IV
1. All of the above plus Hospice certification OR ICD-10 coding certificate.

D. Essential Skills
1. Phlebotomy proficient.
2. Understanding of basic medical terminology.
3. Ability to accurately perform a head-to-toe assessment on patients of all ages.
4. Ability to understand and carry out written, oral, and/or graphic instructions.
5. Ability to accurately perform intermediate math, including addition, subtraction multiplication, and division.
6. Ability to read, analyze, interpret, and apply general business periodicals, professional journals, technical procedure or government regulations.
7. Ability to write accurate documentation, reports, and procedures.
8. Ability to speak effectively on a one on one and small group situation to patients, visitors, and other employees; and to convey health education to individuals.
9. Ability to comply with procedures for infection control and asepsis, and medical waste disposal.
10. Knowledge of med/surg equipment, supplies, and instruments; and the ability to properly use them.
11. Ability to interact with the public in a professional and positive manner.
12. Understanding of and ability to use proper body mechanics to prevent injury to self.
13. Establish and maintain effective work relationships with those contacted in the performance of your duties.
14. Understanding of the principle, practices, and standards of nursing as applied in home health and hospice care.
15. Understanding of pharmaceuticals prescribed for patients of all ages.
16. Plan, assign and supervise work of employees performing in home health care.
17. Ability to set priorities and goals.
